Many patients will fail treatment or have a recurrence, with resistance to chemotherapy being one of the main reasons for this worse outcome. Current chemotherapeutic modalities in uterine cancer include platinum-based and taxane-based chemotherapy; however, resistance to chemotherapy is a challenge that limits the efficacy of these agents in many patients. In this chapter, we discussed the current understanding of chemoresistance mechanisms in uterine cancers and the current and future approaches to overcome the resistance to chemotherapeutic agents. Primary molecular mechanisms of chemoresistance to platinum-based and taxane-based chemotherapy in uterine cancer involve DNA repair mechanisms, efflux pumps, and survival pathways (e.g., PI3K/AKT pathway and MAPK pathway). Multiple targeted drugs have been investigated, primarily in combination with chemotherapy, including AKT inhibitors, PARP inhibitors, mTOR inhibitors, and EFGR inhibitors. More studies will be needed to optimize the use of these drugs. Additionally, multiple mechanisms and targets are being currently investigated, including microRNA, leptin signaling pathway, and others." @default.
