A lot has happened since then, and now, with the encouragement of our current Editor-in-Chief, we are proud to present this special issue of Ophthalmology on IOLs. We have devoted 140 pages to this 6-mm–round and 13-mm–long piece of plastic weighing approximately 20 mg. The IOL is arguably the most life-changing and innovative implant in the history of medicine. Revolutionary in its introduction, the IOL has evolved from a treatment for aphakia to refractive correction and now to spectacle independence. Considering that the IOL implant remains in patients’ eyes for a lifetime, the planning, selection, and surgical implantation are of significant consequence. Understandably, this is a complex decision-making process that requires up-to-date knowledge and evidence to provide optimal patient outcomes. A search using “intraocular lens” on the MEDLINE database turns up approximately 31 000 results, with well more than 1000 publications each year over the last decade. A Google search shows 11.6 million hits. With this in mind, “The IOL Issue” is designed to provide a comprehensive, contemporary review of IOL technology and practice for the clinician. Each article in this collection is intended to present current best evidence as interpreted by a group of international experts. Foundational basics include IOL design and modern-day IOL calculations. Astigmatism management and presbyopic IOL approaches cover advanced refractive topics. Intraocular lens selection and diagnostics provide surgeons with a patient-centered approach, and considerable time is also spent on IOL complications and complex fixation strategies. This collection can be read virtually from cover to cover or by individual articles of interest. Most of all, clinicians will find this a helpful resource while navigating the many options, approaches, and complexities of IOLs. It is likely Harold Ridley never dreamed that modern-day IOLs would serve as a bridge between cataract and refractive surgery, and that nowadays more time is often spent on preoperative IOL planning than cataract surgery itself.
