Significant shifts in extracellular fluid calciumconcentration can have adverse effects on thesephysiological functions. The extracellular concentration of calcium is normally affected by intermittentchanges in calcium absorption in the gut, continuousmineral turnover in bone, and calcium losses in theurine. Extracellular calcium levels are set within a verynarrow range by the concerted action of regulatory‘‘calciotropic’’ hormones on calcium handling by thegastrointestinal tract, bone, and kidney. In childrenand adolescents, maintenance of adequate calciumbalance is particularly important because boneaccretion and growth are closely linked to calciumavailability. Children require an adequate supply ofcalcium to sustain bone mineralization and achievean adequate peak bone mass without disrupting thetight regulation of extracellular calcium. Abnormalities in the organs that express calciotropic hormones,the abnormal function of these hormones or the failureof the gastrointestinal tract, bone or kidney to handlecalcium properly can cause either hypo or hypercalcemia. This chapter will describe the physiology ofcalcium homeostasis, the clinical disorders linked toalterations of calcium concentration in the extracellular compartment, their evaluation and treatment." @default.
