- W32011193 abstract "Extensive proteolytic degradation of articular cartilage is a characteristic feature of joint destruction in rheumatoid arthritis (RA). This prolonged and often inexorable process is generally irreversible and is commonly associated with invasive, hypertrophied synovial pannus tissue comprised of synoviocytes and various inflammatory cells. The histological evidence for cartilage destruction suggests that many degradative mechanisms are possible. The enzymes involved in cartilage degradation, of necessity, relate to the cells that occupy erosion sites. Whereas metalloproteinases are the predominant enzymes of mesenchymal cells such as synovial fibroblasts and chondrocytes, the serine proteinases are more commonly expressed by granulocytes such as polymorphonuclear neutrophils and mast cells. Macrophages and mast cells not only have a broad repertoire of degradative enzymes but also have the potential to produce the pro-inflammatory cytokines and mediators that stimulate proteinase expression by neighboring cells. While macroscopic patterns of cartilage erosion may be recognized, these are often quite different for the histopathology of large and small rheumatoid joints and are often confounded by the specific stage of the disease process. Cartilage loss reflects an imbalance in both catabolic and reparative processes, especially because cartilage matrix does not appear to be replenished with its original properties." @default.
