- W3202643952 abstract "Radiosynovectomy (RSO) is an established therapeutic modality for treatment of arthritis of various origins. For knee joints, main indications are arthritis by rheumatologic diseases, hemophilic arthritis, osteoarthritis, post-traumatic or post-therapeutic arthritis and arthritis in knees with endoprosthesis. In comparison with glucocorticoid administration, RSO provides higher response rates and long-term results. In comparison with arthroscopic synovectomy, RSO provides similar response but lower relapse rates and post-therapeutic morbidity much smaller. The procedure of RSO is similar to an intra-articular administration of glucocorticoids. Arthroscopic synovectomy and RSO can be synergistic in selected patients, for example with pigmented villonodular synovitis. Before performing RSO it is essential diagnose the arthritis. Therefore patients receive clinical examination, ultrasound and two-phase bone scan or alternatively MRI. The intraarticular puncture of knee joints takes place under sterile conditions according to current standards of hygiene. Correct intraarticular position of the needle is highly important. Thus fluoroscopy should be use every time for successful RSO. After RSO, patients receive scintigraphic control of radiocolloid distribution, immobilization of the treated joint with incapacity of work for 2 days, and post-therapeutic evaluation by clinical examination and bone scan. Severe side-effects are very rare, like infectious arthritis, radionecrosis, pulmonary artery embolism or thrombosis. Mild side-effects can occur, like increased post-therapeutic pain or swelling. Radiation exposure of the joint is the therapeutic goal, while radiation exposure of the remaining body is minimized by safe intra-articular administration, use of nanocolloids of proper size and post-therapeutic immobilization. However, radiation protection is important for medical staff. There is no evidence that single or repeated RSO may lead to radiation-based long-term damage. In summary, RSO is an effective and safe procedure with minimal post-therapeutic morbidity in patients with arthritis in larger joints." @default.
