This chapter proposes a series of tests that define a radiolabeled ligand as both a receptor-binding ligand and a sensitive measure of receptor change. The scope of the problem falls into two distinct categories: the development of a radiotracer that binds preferentially to a chosen receptor after intravenous injection and the development of a pharmacokinetic scheme that allows the change in receptor concentration to be detected with accuracy and sensitivity. The greatest strength of nuclear medicine lies in its ability to detect and quantitate biologic function rather than fixed anatomical properties. These functional studies can be divided into two categories: determination of flow and determination of a biochemical interaction, such as receptor binding. There have been many suggestions on how to systematically evaluate a putative receptor-binding radiotracer.
