- W3205578565 abstract "We read with great interest the March 2021 article entitled “Does the Textured-Type Tissue Expander Affect the Outcomes of Two-Stage Prosthetic Breast Reconstruction? A Propensity Score Matching Analysis between Macrotextured and Microtextured Expanders,” by Lee et al.,1 aimed at evaluating possible impacts of the type of tissue expander on the long-term outcomes of two-stage implant-based breast reconstruction. The authors presented a retrospective review of 1391 cases using two different types of tissue expanders, through multivariable analyses and propensity score matching, and concluded that different types of tissue expanders might have an impact on the results of two-stage implant-based breast reconstruction. Although some other articles in the literature studied the controversial advantages and disadvantages of these two tissue expanders,2,3 few kinds of research included the clinical outcomes. We compliment the authors on their excellent efforts in this respect, and would also like to express some considerations regarding the article. First, to reduce the possible heterogeneity, the article adopted the method of propensity score matching. A total of 552 cases were included, 276 in each group, but of all 1391 cases, 1109 cases used Siltex microtextured tissue expanders (Mentor Worldwide, LLC, Irvine, Calif.), and only 282 cases used Biocell macrotextured expanders (Allergan, Inc, Dublin, Ireland). There may still be bias in the matching process. Therefore, we recommend that the number of the cases should be enlarged to make the results more credible. Besides, the article also reported the incidence of complications after the second-stage operation, such as severe capsular contracture, implant malposition, and so on, but the short follow-up time after the second-stage operation would not be enough for evaluating long-term outcomes, so we suggest that the follow-up time be extended to obtain a more accurate complication rate. By the way, there are also some questions in this article. First, in the Results section of the Abstract, the authors state that a total of 1294 patients and 1391 cases were included. However, in the Results section of the article, it became 1324 patients and a total of 1391 cases. We failed to find the details on why the other 30 patients were excluded. Also, in the Statistical Analysis section of the Patients and Methods section, the authors point out that a total of 11 patients and operation-related variables were used for logistic regression analysis, but it included only 10 variables, such as age, body mass index, smoking, and so on. We are also interested in the one remaining variable. We are very grateful to the authors for carrying out this study, which has brought us to the understanding that different tissue-expander types might influence long-term outcomes of two-stage implant-based breast reconstruction. We also hope to see further prospective studies with larger sample sizes as research progresses in this field.
