To determine the rates of adverse outcomes (preterm premature rupture of membranes (PPROM); chorioamnionitis) following transvaginal cervical cerclage for cervical insufficiency in our cohort. A retrospective analysis of electronic medical records was undertaken of all women who underwent transvaginal cervical cerclage at Liverpool Hospital, a busy tertiary referral centre in Sydney, Australia, between January 2016 and December 2020. Multiple pregnancies were included. Exclusion criteria included: loss to follow-up, no follow-up transvaginal ultrasound scan (USS) and preterm delivery for an independent indication. A total of 55 patients were included in the study. The median time from cerclage placement to delivery was 115 days (range 8-192), with 7.5% and 13.2% of cases delivering within 14 and 30 days, respectively. Of all deliveries, 39.6% occurred prior to 37 weeks of gestation, 34.0% prior to 34 weeks and 20.8% prior to 28 weeks. PPROM and chorioamnionitis occurred in 24.5% and 21.2% of cases, respectively. On follow-up USS, 13 (24.5%) had a cervical length below the cerclage of <10mm, and 40 (75.5%) had a length > 10mm. There was no significant difference between these groups in terms of time from cerclage insertion to delivery (either <14 or <30 days), or gestational age at delivery <37 weeks, <34 weeks or <28 weeks. Data regarding the impact of other cervical characteristics (total length; length above cerclage; funnelling) is due to be finalised in July. Preliminary conclusions: PPROM and chorioamnionitis are common complications of transvaginal cervical cerclage in our population, and patients should be counselled appropriately. A short cervical length below the cerclage is not associated with a significantly higher risk of preterm birth." @default.
