- W3206218459 abstract "Fetal anemia secondary to incompatibility between fetal and maternal blood types can result in hydrops and demise. Intrauterine transfusions (IUT) have been performed successfully either by intraperitoneal, intravenous or a combination of techniques. The methods chosen vary based on physician experience and gestational age at the time of presentation. Our objective was to determine the practice patterns amongst fetal centres. A survey was sent to 13 fetal centres from eight countries with a long-standing experience performing IUTs. The survey was online and consisted of ten questions, addressing the experience of the physician, the centre's preferred methods of transfusion, fetal surveillance and timing of delivery. Differences were noted amongst centres with regards to location where IUT was performed, use of maternal anesthesia and hematologic tests performed with the fetal blood with each procedure. Over 50% of the centres performed IUTs in the operating room, while the remaining did the procedure either in a room in the clinic or close to the operating room. No maternal anesthetic was used in 31% of centres, oral or intravenous (IV) sedation was used in 31% and a combination of local with either oral or IV sedation was used in 38%. The type and timing of IUT and delivery were similar. Majority of the centres (84%) performed intravenous transfusions. Intraperitoneal and intracardiac transfusions were performed at 2 centres for very early cases. The last transfusion was performed at 34–35 weeks in 85% of centres and 77% electively delivered their patients at 37 weeks. Location of procedure, anesthetic coverage and surveillance were different across centres. The similarities include timing of the last transfusion and delivery with most centres preferring to deliver at term. A detailed assessment on the complications, interval between transfusions and delivery on the perinatal outcomes should be performed to determine if these differences in practice have any potential neonatal effects." @default.
