- W3207560506 abstract "To evaluate the usefulness of late third trimester fetal Doppler as a predictor of language development during the first 30 months of life. A prospective cohort of 61 low-risk singleton pregnancies with normally grown fetuses was recruited. Fetal Doppler indexes in the umbilical artery (UA), middle cerebral artery (MCA) and cerebroplacental ratio (CPR) were recorded at 36–40 weeks of gestation. The Bayley Scale of Children's Development (Bayley III), the 3rd edition of Ages and Stages Questionnaires (ASQ-3TM) and the MacArthur-Bates I and II Communication Skills Development Questionnaires (SCDI) were then applied within the first 30 months of age. Based on their language performance, children were allocated in the Normal Score (NS) group when all scores were normal, and Low Score (LS) if at least one score was abnormal in any of the aforementioned tests. The relationships between fetal Doppler and infant language development were evaluated by linear and logistic regression analysis. 63.9% (39/61) and 36.1% (22/61) children were allocated in the NS and LS group, respectively. LS children showed significantly lower CPR values (-0.75 vs. 0.20 z-scores, p < 0.001) than those with NS. Fetal CPR Doppler values showed a positive correlation with infant language scores including vocabulary production (R2 = 0.211, p < 0.001), vocabulary comprehension (R2 = 0.115, p = 0.053), gestures and actions (R2 = 0.167, p = 0.018), use of words (R2 = 0.208, p = 0.015) and Bayley language index (R2 = 0.245, p = 0.016). In comparison with fetuses with normal CPR Doppler, cases with decreased CPR (< 5th percentile) showed significantly higher risk of abnormal language scores in communication (ASQ-3TM), with an odds ratio (OR) of 21.2, [IC 95%]1.62 – 276.9, p = 0.02). Fetal Doppler within the last weeks of pregnancy could be of potential utility to predict infant language development. Prenatal awareness allows early detection and treatment." @default.
