- W3207679654 abstract "To study the efficacy of uterine artery Doppler between 11–14 weeks in predicting hypertensive disorders of pregnancy (PIH) and intrauterine growth restriction (IUGR). The study was conducted at Bangalore Baptist Hospital, a tertiary care centre over a period of 2 years from January 2015 to December 2017. Inclusion criteria: All pregnant women between 11–14 weeks with singleton pregnancy. Exclusion criteria: women with anomalous babies, multiple pregnancies and essential hypertension. After taking an informed consent, women meeting the inclusion criteria were scanned by trained sonologists. Uteroplacental circulation is measured by bilateral uterine artery Doppler indices namely resistance index (RI) and pulsatility index (PI). Increased resistance to flow in uterine artery is associated with appearance of diastolic notch and increase in Doppler indices. Women with RI and PI values above 95th centile are taken as test positive and were followed up till delivery for development of PIH and IUGR. 571 women were included in the study of which 28 developed PIH and 14 developed IUGR. Sensitivity of mean PI in prediction of PIH and IUGR was 17.85%, and 25% and specificity was 94.47% and 94%. Sensitivity of mean RI in prediction of PIH and IUGR was 32.14% and 21.42% and specificity was 96.13% and 49.38%. Sensitivity of early diastolic notch in prediction of PIH and IUGR was 71.42% and 75% and specificity was 71.42% and 49.38%. This study showed that mean PI is a fair indicator for prediction of PIH but not for IUGR whereas RI is a poor predictor for both PIH and IUGR. First trimester uterine artery Doppler is not an effective screening tool as it has high specificity but low sensitivity however, it can be used in selected high risk patients to enable early interventions such as starting low dose aspirin." @default.
