- W3207800859 abstract "To evaluate the association and predictive value of the MPI and CPR in predicting adverse perinatal outcomes in AGA, SGA and FGR fetuses. This prospective cohort observational study was carried out in the Department of Obstetrics and Gynecology at AIIMS, New Delhi, between January 2018 to January 2020. Singleton pregnancies with confirmed dating by first trimester scan and with normal anatomy scans were recruited prospectively from antenatal clinic after 24 weeks of gestation. We recruited 103 singletons constituting 48 AGA, 11 SGA and 44 FGR fetuses. These pregnancies were subjected to biometry and Doppler scans and were further classified as AGA (AC/EFW > 10th centile), SGA (AC/EFW - 3rd to 10th centile with no Doppler abnormalities) and FGR (AC/EFW < 3rd centile or 3rd to 10th centile with abnormal Dopplers). At each contact the patient underwent a scan for growth and liquor with Doppler studies which included umbilical artery PI, middle cerebral artery PI, ductus venosus PI, aortic isthmus PI and CPR was calculated. Fetal echocardiography was done to calculate modified myocardial index as described by Hernandez et al. UA PI, AoI PI and MPI were higher while MCA PI and CPR were lower in FGR group. The OR for composite adverse perinatal outcome in FGR group for MPI > 0.47 is 3.48 (95% CI: 0.99–12.24, p value < 0.05) with sensitivity and specificity of 65% each. The OR for composite adverse perinatal outcome in FGR group for CPR < 1.67 is 11.08 (95% CI: 2.62–46.83, p value =0.001) with sensitivity of 70% and specificity of 86% each. In the FGR group when the cut off values for MPI was taken > 0.47 and CPR< 1.67; using logistic regression analysis, it was found that the OR (odds ratio) was 58.5 (95% CI: 4.58–746.57, p value =0.002) with sensitivity of 95% and specificity of 56.5%. MPI, CPR, and IRT were independent predictors of composite adverse perinatal outcome in the FGR Group. MPI in conjunction with CPR could be used together to predict adverse perinatal outcome in FGR." @default.
