It has been estimated, that between 10 and 15% of all foot and ankle fractures fulfil the criteria of complex foot and ankle trauma. Complex trauma of the foot and ankle is a predictor of unfavorable outcome in polytraumatized patients. The treatment objective of complex foot and ankle trauma is the restoration of lower limb function which ideally should be painless, stable and functional whilst avoiding complications such as infection. Initial management of complex injuries of the foot requires recognition and treatment of compartment syndromes, early soft-tissue coverage, and rigid skeletal stabilization to enhance soft-tissue healing. Early total care should be adopted when both the patient's systemic condition and the local conditions are adequate to support the duration of the procedures. Staged treatment may be required in patients that have suffered life threatening injuries or were soft tissue cannot support duration of surgery. Site specific details and patient characteristics need to be appreciated when dealing with complex foot ankle injuries especially when amputation or reconstruction are discussed. Die Literatur zeigt eine hohe Koinzidenz zwischen komplexen Fuß- und Sprunggelenktraumata und Polytrauma (bis zu 52% der Fälle). Es wird geschätzt, dass zwischen 10 und 15% aller Fuß- und Sprunggelenkfrakturen die Kriterien eines komplexen Fuß- und Sprunggelenktraumas erfüllen. Ein Komplextrauma des Fußes oder des Sprunggelenks ist ein Prädiktor für ein ungünstiges Resultat beim polytraumatisierten Patienten. Das Behandlungsziel bei komplexen Fuß- und Sprunggelenktraumata ist die Wiederherstellung der Funktion der unteren Extremitäten, die idealerweise schmerzfrei, stabil und funktionsfähig sein sollte, während Komplikationen wie Infektionen vermieden werden. Das initiale Management komplexer Fußverletzungen erfordert die Erkennung und Behandlung des Kompartiment Syndroms, eine frühzeitige Weichteilabdeckung und präliminare Frakturstabilisierung, um die Weichgewebeheilung zu verbessern. Wenn sowohl der systemische Zustand des Patienten als auch die lokalen Bedingungen ausreichen, um die Dauer der Eingriffe zu unterstützen, sollte eine frühzeitige umfassende Versorgung erfolgen. Bei Patienten, die lebensbedrohliche Verletzungen erlitten haben oder bei denen das Weichgewebstrauma längere Eingriffe nicht zulässt, kann eine mehrzeitige Behandlung erforderlich sein. Bei komplexen Fuß- und Sprunggelenkverletzungen, insbesondere wenn Amputationen oder Rekonstruktionen diskutiert werden, müssen die individuellen Gegebenheiten und Ansprüche des Patienten berücksichtigt werden." @default.
