- W3208615561 abstract "Introduction: The incidence of pancreatic cancer (PDAC) increases with age. More elderly patients aged ≥80 are presenting with resectable PDAC for consideration of treatment. However, increasing age is associated with higher rates of co-morbidity, frailty, and reduced functional reserve, causing some concern for undertaking a pancreaticoduodenectomy. We aimed to assess differences in pathways and outcomes for patients planned for pancreatoduodenectomy for PDAC stratified by age. Methods: A retrospective national study undertaken by 26 specialist pancreatic resectional centres in the United Kingdom. All adult patients listed for pancreatoduodenectomy for suspected PDAC were included. Baseline demographics, neo-adjuvant chemotherapy regimens, length of pathway (days from MDT to surgery), number and type of investigations, interventions (ERCP, PTC) and associated complications were recorded, in addition to the operation performed, and 12-month survival. Results: 1709 cases were entered from 2017-2020. 104 patients were excluded due to incomplete data entry. The median age was 68 years (IQR 59-73). 105/1642 (6.4%) patients were aged ≥80 years. There was no significant difference in the length of diagnostic pathway between age groups (56 days in ≥80 years vs 49 days, p=0.399), number or type of investigations, or number of MDT discussions (median 2, IQR 1-3). One quarter of patients had a pre-operative cytological/histological diagnosis (24.8% vs 27.6%, p=0.864). Elderly patients had a higher performance status (1 vs 0, p<0.001). At 12-month follow-up, 33 patients <80 years had died, compared to 370 patients <80 years, giving a 1-year survival rate of 68.6% vs 86.0% (p=0.076). Conclusion: Elderly patients are being offered potentially curative resections in the United Kingdom. There were no significant difference in the diagnostic work-up for these patients, but a slight decrease in 12-month survival. Further analysis is required to understand the reasons for marginally poorer survival in this group." @default.
