Despite the frequent hospitalizations for pancreatitis, inpatient smoking cessation counseling is often overlooked in clinical practice. The purpose of this pilot study is to improve the inpatient management of pancreatitis by adapting the evidence-based Rochester Model (RM) of smoking cessation counseling. Methods: The Rochester Model (RM) of smoking cessation is an evidenced based intervention that is comprised of an inpatient counseling followed by 4 post-discharge counseling phone encounters and 3 outcome phone encounters (4 weeks, 3 months, and 6 months). This program has demonstrated outcome measures of 33% quit rate at 6 months for patients who completed all post-discharge phone encounters with a 9% overall quit rate at 6 months. An inpatient consult service comprised of medical students was developed to screen for active smoking in all patients admitted to our tertiary care hospital medicine teaching service with pancreatitis. As per the RM, eligible patients were assessed for interest in smoking cessation or noninterest in the study. Enrolled patients received 2 treatment calls from the NYS quitline, 2 treatment calls from their assigned patient educator and 3 outcome calls at intervals of 4 weeks, 3 months, and 6 months post discharge to assess smoking status at the time of each call. Results: 45 eligible patients were screened between July 21st and December 6th, 2020. 19 patients enrolled and were followed for 6 months. 6 patients completed all counseling. The outcomes of adapted Rochester Model smoking cessation for pancreatitis are shown in Figure A. Smoking cessation rates for those who completed counseling were 33.3%, 50%, and 50% at 4 weeks, 3 months, and 6 months, respectively. The general pancreatitis cohort had smoking cessation rates of 11%, 16%, and 21%, respectively. The smoking cessation rates at 3 months and 6 months amongst patients with pancreatitis who completed counseling were superior compared to the cessation rates of the original Rochester Model accrued from January 2013 and August 2018. Conclusion: Inpatient smoking cessation counseling for patients with pancreatitis can achieve significant quit rates. Decreased smoking was noted in patients who failed to quit. The Rochester Model is an evidence-based inpatient nicotine dependence treatment program that can be adapted to patients admitted with pancreatitis for efficient counseling, reduced burden on healthcare providers and higher quality of care.Figure 1.: Smoking cessation rates for the original Rochester Model cohort (completed counseling vs total) and pancreatitis cohort (completed counseling vs total) who received the adapted Rochester Model inpatient smoking cessation counseling." @default.
