- W3209595087 abstract "Introduction: Personality Disorder (PD) and its subtypes arise from inflexible and maladaptive traits that cause distress or functional impairment, and are strongly linked with deliberate foreign body ingestion (DFBI), a psychopathological self-injurious disorder. This study provides insight into the removal of foreign bodies in PD vs non-PD cases. Methods: This review searched electronic databases with MeSH terms and Boolean logic adhering to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. It includes peer reviewed case reports, published in English from 2015 to mid-2021, which declare a clinical diagnosis of PD in adults (≥18 years), endoscopy as initial treatment, and description of the number and type of foreign bodies (FBs). Endoscopy was considered successful if the FBs were removed, and unsuccessful when converted to surgical removal. Cases that did not use endoscopy (e.g. went directly into surgery) were excluded. The Joanna Briggs Institute (JBI) critical appraisal tool was utilized to assess eligible studies. Results: Seven case reports with PD and 48 case reports without PD were included. PD cases were older (36.86 vs 34.54 years, p=0.60), with a greater mean number of concomitant psychiatric conditions (μ=3.00 vs 1.23, p< 0.001). The most common PD classification was Borderline Personality Disorder (PD Cluster B) (n=6, 86%); 1 case report gave no specific PD classification. PD cases ingested more large wood items >3cm (e.g. pencil) (μ=2 vs 0.04, p< 0.01), sharps/blades (μ=9.43 vs 1.29, p< 0.05), and batteries (μ=4.43 vs 0.42, p< 0.01), and reported more endoscopic attempts to remove FBs (μ=1.29 vs 1.04, p< 0.05). The most common endoscopy performed in both groups was upper gastrointestinal endoscopy (86% and 90%, respectively). There were no significant differences between rates of successful endoscopic FB removal in PD and non-PD cases (μ=0.86 vs 0.67, p=0.32). There was only 1 reported surgery (gastrotomy) among PD cases, which occurred in the case without PD classification. The most common surgeries for non-PD cases were gastrotomy (n=9, 31%) and laparotomy (n=8, 28%). There were no mortalities. Conclusion: In PD cases, there is a higher incidence of ingestions of certain readily accessible FBs and more endoscopies performed, than non-PD cases. Further research is needed to know economic implications of PD in DFBI management, and whether certain Clusters of PD (e.g. Cluster A vs B vs C) may be more predisposed to DFBI than others." @default.
