- W3210725315 abstract "Introduction: Acute pancreatitis (AP) is a highly inflammatory state that is associated with significant morbidity and mortality. There is limited data on the natural history of AP in African American (AA) patients. The aim of our study was to assess the disease characteristics and outcomes in AA patients with AP and compare with non-AA patients. Methods: We retrospectively analyzed patients with a confirmed diagnosis of AP admitted to our institution from 2008-2018. We compared demographic characteristics, comorbidities, severity of AP and outcomes of AP between AA and non-AA patients. Continuous variables were represented as means and standard deviations. These were compared between the study and control groups using the t-test or Wilcoxon rank-sum test, as deemed appropriate. Categorical variables were represented as frequencies (percentage) and analyzed using the 2-way chi-square test to compare the 2 groups. Statistical significance was assigned at 0.05. Results: Out of 878 patients with AP, 187 patients were of AA ethnicity. Apart from a higher proportion of AA patients being men (58.3% vs 49.4%; P=0.03), there were no significant differences between groups (Table). Alcohol was a significantly higher cause for AP in AA patients (31.2% vs 17.4%, P=0.013) who also had greater use of alcohol (45.5%vs 34.2%, P=0.006) and tobacco ((38.5% vs 24.6%, p< 0.001). A higher proportion of AA patients had mild disease (83.4% vs 73.7%, P=0.004). The average time to refeeding was significantly shorter in AA patients (1.89±2.39 vs 2.38±2.50 days; P=0.016). There were no other significant differences in patient outcomes (Table). Conclusion: Although some demographic and clinical features of AA patients with AP differ from non-AA patients, there is no difference in outcomes including length of stay, ICU stay, mortality and readmissions in AA patients compared to other ethnicities. Further work is needed to identify areas of healthcare disparities that may exist in management patients of different ethnicities.Table 1.: Comparison of demographic characteristics, comorbidities and outcomes between African American and non-African American patients with acute pancreatitis" @default.
