- W3211323223 abstract "Introduction: SARS-CoV-2 infection can lead to a variety of manifestations beyond viral pneumonia. Recent data has implicated liver disease as a part of the disease presentation. Studies have shown AST and ALT elevations in 14%-53% of COVID-19 cases. However, no retrospective or prospective studies currently exist to evaluate the long-term outcomes of these abnormalities. To determine this relationship, we are conducting prospective and retrospective studies to evaluate the COVID-19 outcomes associated with abnormal liver tests. Methods: Prospective study: We plan to include patients diagnosed with PCR confirmed COVID-19 between April 2021 to December 2021 with abnormal liver function tests in the absence of underlying liver disease. We anticipate a sample size of 200 and have enrolled 20 patients thus far. We plan to follow them with liver profiles every three months. Retrospective study: We will enroll patients admitted from January 2020 to December 2020 with PCR confirmed COVID-19. We are analyzing 631 patients with abnormal liver profiles to assess for adverse outcomes during hospitalization. Adverse outcomes will be defined as morbidity, mortality, need for intubation, length of hospital stay, critical care admission, or one year readmission. Results: Dichotomous and categorical variables will be compared by using chi-square analysis and Fisher exact test. Statistical significance will be defined as a P-value of < 0.05. We are currently analyzing and generating data with plans to publish. Conclusion: Available evidence has shown that SARS-CoV-2 infects intestinal cells through ACE2 receptors and some patients with COVID-19 exhibit viral RNA in their feces and blood. This suggests the virus may reach the liver via the portal circulation and Kupffer cells might attempt to clear the virus and produce local inflammation. The liver may also be affected by the hypoxia and cytokine storm that follows SARS-CoV-2 infection. The goal of these two studies is to evaluate and monitor abnormal liver function tests in COVID-19 and correlate this to clinical outcomes.Figure 1.: Consort diagram of the study plan and data collection sheet." @default.
