- W3211871851 abstract "Explaining below-level pain following spinal cord injury (SCI) has been hampered by investigating changes in neural processing that occur caudal to the spinal infarct. This seemingly logical approach relies on an assumption that abnormal, below-level nociception propagates rostral to the spinal injury to activate thalamic and cortical regions that process the conscious perception of pain. However, nociceptive projection from below- to above-level can be prevented by SCI. Also, this approach does not consider that below-level pain develops following transection of the pathway for rostral transmission of nociception from the spinal cord—the spinothalamic pathway. Thus the mechanisms of below-level SCI pain depend upon neural adaptations at and above the spinal injury, not below it. This review focuses on long-term experiments involving humans, monkeys, and rats following spinothalamic cordotomy. Cordotomy turns out to be an appropriate, across-species model for development of below-level pain following SCI." @default.
