- W3215444851 abstract "Transvenous phrenic nerve stimulation provides a mechanism to combat atelectasis-driven lung injury in sedated patients unable to breathe spontaneously, by delivering a targeted stimulus to the diaphragm. This neurostimulation contracts the diaphragm to counteract the caudal pressure from abdominal contents and promotes a more physiologically normal distribution of tidal volume, helping to keep alveoli open. A preclinical study of ~50 kg deeply sedated pigs, ventilated with lung-protective volume control ventilation at 8 ml/kg, was conducted for 50 hours. Temporary transvenous diaphragm neurostimulation (TTDN) was delivered in synchrony with inspiration on every breath to reduce the ventilator pressure-time product by 15-20% for the TTDN group. TTDN on every breath (MV+TTDN100%) was compared to protective mechanical ventilation only (MV). The change in end-expiratory lung volume (EELV) was measured using electrical impedance tomography (EIT). EIT belts remained in place for the duration of the experiment and animal fluid balance was very tightly controlled. TTDN plus lung-protective ventilation had a larger effect on the ventral lung regions, where hydrostatic pressure transference from the abdominal contents to the pulmonary tissue is lower thus requiring less oppositional force.The gentle diaphragm contractions provided in this study also prevented EELV decline in the dorsal regions, however to a lesser extent." @default.
