- W3215534747 abstract "<b>Aim:</b> To evaluate the effect of non-invasive ventilation (NIV) on cerebral, respiratory and peripheral oxygenation when compared to SHAM ventilation during high-intensity exercise (HIE) in patients with coexisting chronic obstructive pulmonary disease (COPD) and heart failure (HF) <b>Methods:</b> This is a randomized, double blinded, sham-controlled study involving 14 male COPD-HF. On separate days, patients performed incremental cardiopulmonary exercise testing (CPET) and two constant-work rate tests (80% peak of CPET load) receiving NIPPV or SHAM until the limit of tolerance (Tlim). During HIE, oxyhemoglobin (O2Hb) and deoxyhemoglobin (HHb) were assessed using near-infrared spectroscopy on: vastus lateralis (VL); external intercostal muscle (EI) and prefrontal region (PF). <b>Results:</b> NIV associated with HIE caused a significant increase in Tlim (NIPPV: 129±29 vs SHAM: 98±29s, p=0.01) and SpO2 (NIPPV: 94±3 vs SHAM: 92±5%, p=0.03). In PF, EI and VL, NIPPV provided a lower drop in (O2Hb) (p<0.05) and a lower increment (HHb) response (p<0.05) from the half of the test (60%) when compared to SHAM ventilation (Figure 1). <b>Conclusion:</b> NIV as an adjunct of a HIE was able to provide respiratory muscle unloading with greater oxygen supply to the peripheral muscles, sustaining longer exercise time in COPD-HF patients.
