- W3216406522 abstract "Background: The causality relation between obesity,A/asthma and OSA is unclear. The children with OSA may have an inconsistent response to the recommended treatments(adenotonsillectomy, continuous positive airway pressure(CPAP)), indicating different phenotypes and not one treatment fitting all. They may even increase their Body Mass Index(BMI)on CPAP. Apnea-hypopnea index(AHI) evaluation alone seems insufficient to predict the need and response to treatment. Objectives: To evaluate a possible response pattern of children with OSA and different PG parameters to the currently recommended treatments and to the Allergic Immunotherapy(AIT)if applicated. Methods: In 78 children(2-16years)with clinical OSA we had outpatient Respiratory Polygraphy(PG)(cidelec-LX/somnolter jawac)in a longitudinal way during 1.01.2018-30.08.2019(TRUST IT ALL STUDY). We assessed response to treatments along to PG parameters, OSA risk factors, pre-existing asthma,A(dust mites, respiratory, food), age, sex, BMI. Results: Severe OSA(AHI:31,7n/h, RE:62,3%)without associated A ameliorated on CPAP in all PG parameters. Severe OSA with associated A and no adequate A/asthma treatment failed to decrease RE(RE:57,3%, AHI:1)on CPAP, and increased the BMI(+30kg).A treatment/AIT decreased RE and BMI, thus avoiding asthma and obesity.Isolated AHI increasewithout associated RE increase(RE:2%)ameliorated with nasal corticosteroids only. Conclusion: Persistently increased RE in allergic children with OSA on CPAP, maybe a sign of increased obesity risk through sleep fragmentation and stress.RE decrease after A treatment, indicates increased RE as a PG parameter of A or asthma." @default.
