- W3216913310 abstract "<b>Background:</b> The delivery of a high and consistent fraction of inspired O<sub>2</sub> (FiO<sub>2</sub>) is imperative to treat severe acute hypoxemia. <b>Objective:</b> To analyze the FiO<sub>2</sub> delivered by different low-flow oxygen therapy systems (LFOTS) challenged with different O<sub>2</sub> flows and respiratory patterns in an experimental lung model. <b>Methods:</b> An adult lung model ventilated (volume control) simulated different combinations of tidal volume (Vt, 350 or 700mL) / respiratory rate to obtain minute ventilation (VE) of 7, 10.5, 14L/min. The FiO<sub>2</sub> delivered in the lung’s model by nasal cannula (NC), simple facemask (SFM), non-rebreather mask (NRM), and double-trunk mask with nasal cannula (DTM<sub>NC</sub>) tested at O<sub>2</sub> flow of 10 and 15L/min was quantified. The SFM, NRM and DTM<sub>NC</sub> were sealed tight on the virtual airway opening. Their performance was also tested with a standardized leak. <b>Results:</b> All LFOTS performed better with O<sub>2</sub> flow of 15L/min compared to 10L/min. Within each device, there was a significant effect of VE (p=0.02) except for the NRM sealed tight (p=0.06). The higher the VE, the lower the FiO2. Vt had little impact. In the leaked condition, the average delivery of FiO<sub>2</sub> significantly dropped with the SFM (-4%) and the NRM (-13%), but not the DTM<sub>NC</sub> (0%). The DTM<sub>NC</sub> outperformed all other LFOTS, especially in leaked condition <b>Conclusions:</b> The FiO<sub>2</sub> sampled in the lung model with the DTM<sub>NC</sub> were higher than those obtained with other LFOTS, especially when leaks were present." @default.
