- W3217113852 abstract "In the current context of the COVID-19 pandemic, oxygen-requiring patients have a strong impact on hospital oxygen capabilities. Few studies have shown that the use of the Double Trunk Mask (DTM) improves arterial oxygen pressure (PaO<sub>2</sub>) for patients with acute hypoxemic respiratory failure. In these patients, Duprez F. et al. (Duprez F, Respir Care. 2019 Aug;64(8):908-914) showed an improvement of PaO<sub>2</sub> from 68±14 mmHg to 88±23 mmHg (p<0.001) after the application of the DTM, without increasing PaCO<sub>2</sub>. We conducted an observational study since March 2020 on the application of the DTM for 100 patients (mean age 67 Yo, 65% men) with COVID-19 in conventional hospitalization, not eligible for ICU, requiring up to 6L/min of O<sub>2</sub> with nasal cannula or non-rebreathing mask and presenting a SpO<sub>2</sub> lower than 90%. The DTM consists of a normal aerosol mask with two side holes of 22 mm in diameter, into which a 38 cm flexible tube is inserted on each side. This arrangement increases the O<sub>2</sub> concentration and allows removal of CO<sub>2</sub>. After the installation of the DTM, 70% of the patients were responders. We observed an average increase of 4 points in SpO<sub>2</sub>. The DTM allows to decrease O<sub>2</sub> flow to keep SpO<sub>2</sub>>90%. However, we observed 30% non-responders who do not show a decrease of SpO<sub>2</sub>. The application of the DTM is done over the nasal cannula, which keeps a safety continuous oxygenation (during sleep and meals). In patients with COVID-19 receiving oxygen via nasal cannula, DTM allows better arterial saturation for the same oxygen flow. This type of mask, simple to manufacture, can allow to lower the consumption of medical oxygen while maintaining better oxygenation for patients ineligible in ICU." @default.
