- W3217136629 abstract "<b>Background:</b> A systematic exploration of the ventilatory efficiency parameters during a cardiopulmonary exercise testing (CPET) in the lung transplant (LTx) recipient is lacking. <b>Aims and objectives:</b> To evaluate the indices of ventilatory efficiency measured during incremental CPET and their role in exercise limitation of LTx recipients. <b>Methods:</b> Thirty-six bilateral LTx recipients and 13 healthy controls (HC) were enrolled in an observational cross-sectional study. An incremental CPET was performed until exhaustion. Arterial blood gases (ABGs) at rest and peak exercise were also collected. Ventilatory efficiency parameters were measured. <b>Results:</b> LTx recipients showed a significant increase of VE/VCO2 slope [median (IQR) 28,2 (25,6-31,9) vs 23,7 (22,1-25,2), p<0,0001], VE/VCO2 nadir (mean±SD 33,9±4,6 vs 24,4±2,2 (p<0,0001) and VE/VCO2 peak [35,6±4,4 vs 27,9±3,0 (p<0,0001)], and a higher intercept on the VE-axis (4,7±2,0 vs 2,9±1,7 L/min (p=0,0009)] compared to HC. A higher breathing reserve at peak exercise, a lower but preserved physiological fall in VD/VT ratio, the absence of ventilation-perfusion mismatch in terms of P(a-ET)CO2 and A-aO2 at peak exercise were found in LTx recipients. Respiratory exchange ratio at peak of exercise increased in LTx recipients, reaching higher values compared to HC subjects [1,23±0,15 vs 1,16±0,06 (p=0,03)]. <b>Conclusions:</b> Our preliminary results show that the reduced ventilatory efficiency during incremental exercise in LTx recipients is associated with an altered peripheral metabolic pathway but not to wasted ventilation, breathing pattern or gas exchange abnormalities.
