- W331483737 abstract "In recent years there has been an upsurge in the utilization of organic nitrates in clinical medicine. A variety of factors are responsible for the increasing interest in the role of nitroglycerin (NTG) and long acting nitrates. The potent vasodilatory actions of these compounds on the venous and arterial circulation account for the extremely important therapeutic role of nitrates in ischemic heart disease. While the precise mechanisms for ischemia relief in the various angina pectoris syndromes are complex and somewhat controversial, there is little question but that these compounds are extremely effective in the treatment of myocardial ischemia. Sublingual NTG remains the gold standard around the world for immediate therapy of acute ischemic events. Investigators and clinicians have recognized that the unique vasodilatory actions of the nitrates are beneficial in acute and chronic congestive heart failure. The advent of vasodilator or unloading treatment for heart failure in the early 1970’s provided an important new role for the nitrates. These drugs are the only available oral agents that have direct venodilating properties; it is the venous effects of these drugs that make them unusually beneficial in the treatment of congestive heart failure. Recognition that high concentrations and/or large doses of organic nitrates will produce important reductions in systemic blood pressure has resulted in the increasingly widespread use of intravenous nitrates for urgent lowering of elevated blood pressure or for controlled hypotension during delicate surgical procedures." @default.
