- W335303984 abstract "Publisher Summary This chapter discusses dermatomyositis (DM) and polymyositis (PM) syndromes. It discusses a study in which the histochemical and ultrastructural features of idiopathic DM in 30 children and 8 adults and of DM in 5 adults with carcinoma were compared with the features of DM or PM in 16 adults in the overlap group (rheumatoid arthritis, systemic lupus erythematosus, scleroderma, mixed connective tissue disease, and the Sjogren syndrome), 10 adults with idiopathic PM, 8 adults with inclusion body myositis, and 50 children and adult controls. In all children and adults with DM, the abnormalities varied only in degree. Perifascicular atrophy was detected by the NADH dehydrogenase technique as well as by the characteristic degeneration of muscle fibers, all in the same stage of degeneration at the periphery of portions of some fasciculi. The degenerated muscle fibers contained disorganized contractile substance, cytoplasmic bodies, Z band streaming, rod bodies, lipid bodies, and glycogen. The necrosis of muscle fibers in a perifascicular distribution was present as well. With larger zones of involvement, the degeneration was at a stage similar to that in the perifascicular zones. Muscle nuclei within both central and peripheral fibers contained hypertrophied nucleoli, nuclear bodies, and an increased number of perichromatin and interchromatin granules. Dense inclusions imparted the appearance of spotted nuclei. Arterioles, veins, capillaries, and lymphatics contained a mixture of degenerated, regenerated, and necrotic endothelial cells. Tubular aggregates were present in the endothelial cytoplasm of more than 80% of the intramuscular blood vessels and less frequently in pericytes and satellite cells." @default.
