- W33850622 abstract "250 Objectives Osteochondral angiogenesis is present in osteoarthritis (OA) and contributes to synovial hyperplasia, bone destruction, pain and disability. PET imaging of angiogenesis using (18)F-FB-mini-PEG-E[c(RGDyK)](2), [18F]FPRGD2 targeting integrin αvβ3, is appealing for early diagnosis and treatment assessment of OA. We observed the uptake of [18F]FPRGD2 in joint diseases and periarticular changes in patients who had undergone PET/CT for oncologic purpose. Methods We prospectively studied 47 patients included for rectal carcinoma (N=27) or a renal mass (N=20) prior to treatment. PET/CT was performed 60 minutes after IV injection of 300 MBq [18F]FPRGD2. An experienced physician reviewed PET images to estimate the accumulation of [18F]FPRGD2 in joint, periarticular processes and spinal degenerative changes. For 31 patients, FDG PET/CT performed in clinical settings was available for the comparison of FDG and [18F]FPRGD2 uptake. In each joint with abnormal uptake, a volume of interest (1.22 ml) was delineated in the area of highest uptake to estimate the maximum uptake value (SUVmax). Results A total of 157 osteoarticular lesions with [18F]FPRGD2 uptake were detected and the most common accumulation sites were spine zygapophyseal (N=36) and acromioclavicular (N=25) joints and scapulohumeral periathritis (N=20). Out of 101 lesions for which FDG PET was available, 32 (32%) did not show FDG avidity; 11/24 (46%) of spine zygapophyseal joints with [18F]FPRGD2 uptake did not show FDG uptake. Only 1 scapulohumeral process showed FDG uptake without [18F]FPRGD2 uptake. The mean intensity of [18F]FPRGD2 and FDG uptake was similar in the 68 lesions with [18F]FPRGD2 (mean SUVmax 2.6; SD 1.2) and FDG (mean SUVmax 2.6; SD 1.4) uptake. Conclusions We observed [18F]FPRGD2 uptake in OA or periarticular processes. When FDG PET/CT images were available, one third of lesions with [18F]FPRGD2 uptake did not show FDG avidity." @default.
