An overview of psycho dynamic therapy that blends object-relations and ego psychology is provided. This is followed by a discussion of an incarnational approach to integration. This approach conceptualizes the patient's presenting problems from both a psychological perspective and a theological perspective that emphasizes the human need for relationship. Therapist intervention is and intentional in its deliberate use of the therapeutic relationship that serves as the catalyst for change of relational patterns. A case study utilizing this approach is described. As a psychodynamically-oriented Christian psychologist, I have long wrestled with the tensions inherent in such a position. Consider for instance the wariness that many Christians have for anything remotely related to Freud, the more directive approach advocated by many clinicians and theoreticians who write about integration, and the theoretical tension of remaining more a blank slate for patients than a spiritual guide or coach. These tensions have sometimes left me feeling a bit less integrated in my work than some of my colleagues, since I only rarely began a session with prayer or assigned scripture for my patients to memorize. I have come to believe that it is time to broaden our perspectives on what explicit integration means. My own theoretical biases tend toward some amalgam of interpersonal and object relations models. This movement came a bit later in my theoretical development, having been trained in a program that leaned heavily toward cognitive and constructivist models of therapy, followed by a residency in behavioral medicine with its uniquely behavioral (and cognitive-behavioral) approach to clinical problems. My early years as a psychologist were spent practicing cognitive restructuring and relapse prevention. In a medical setting these skills proved quite helpful and I learned to apply them in a number of shorter term interventions for pain management, coping with chronic illness, and dealing with addiction, among other things. In more structured settings with more circumscribed goals, this model served the patient well and I felt comfortable implementing it. As long as the goal was learning a new way of thinking about a problem or providing knowledge the patient did not have, the models with which I was trained were quite effective. However, sometimes the presenting problems were deeper than the level of the patient's cognitions and I longed for a way to touch those deeper needs. On one occasion, I was seeing a high level executive for depression and marital distress. As he began to describe avoiding intimacy and living together only as economic partners and parents, the pain was evident. On his second session, he was quite tearful, telling me Everything's falling apart and I don't know what to do. As we began to talk about a rational plan for how to proceed, he told me You're obviously a nice person, but I don't think this is helping and left the session 30 minutes early. I've often wondered how I might better have spoken to the pain he was experiencing. On another occasion, I had a patient whose presenting problem was his enjoyment of being spanked. Though the patient was unaware of and reported no overt sexuality associated with these spanking sessions, you can imagine the wife's consternation and anger when she discovered this world of her husband. The patient himself was very intelligent, yet could not understand what he called his compulsion. We worked together using cognitive models of therapy for 25 sessions over a period of almost a year before the patient decided that the treatment wasn't helping.
