- W34879236 abstract "Background: Chylous ascites is a rare complication of the gastrointestinal carcinoid. There are two mechanism of its origin. First, the mechanical obstruction by the tumor mass and second, the fibrosis of the surrounding tissue due to overproduction of serotonin. Its presence is very limited course of cancer treatment. Case: We report a case 66-year old man suffering from recurrent diarrhea and ascites. The diagnosis was found by elevated tumor marker Chromogranin A and the elevation of hydroxyindoleacetic acid (5-HIAA) in the urine. A subsequent whole-body scintigraphy 111In Octreoscan confirmed multinodal process with increased somatostatin receptors activity in the wall of the ileum, rectosigmoideum, lymph nodes of retroperitoneum and mesenterium and left supraclavicular area. We performed biopsy from the lymph node of supraclavicular area and there was encountered metastasis of the carcinoid. Start of cytostatic therapy was repeatedly complicated by recurrent massive chylous ascites. The patient underwent only one serie of the palliative chemotherapy. Another procedure was again complicated by chylous ascites, that caused hospitalization at the intenal department and the patient died 4 months after the estimation of cancor diagnosis. Conclusion: Chylous ascites is a very rare complication of gastrointestinal carcinoid. It´s not only a markersign of poor prognosis, but also a complication, that makes difficult to process systematic treatment." @default.
