- W368962446 abstract "Community care is a favored way of supporting disabled and older people in many countries. Arrangements for funding and delivering community – i.e., noninstitutional – services vary between countries and welfare regimes. This article focuses on the development of community care policies and practice over two decades in England. Nevertheless, the themes shaping community care in England – particularly the growing emphases on markets and service user choice – are very similar to priorities in many other countries. Over the past three decades, there have been marked shifts away from hospital-based and institutional care for disabled and older people, toward increased provision of intensive support for people in their own homes. These changes have been accompanied by the wholesale marketization of community care services, with first local authorities and later service users themselves increasingly purchasing publicly funded care from private provider organizations or privately employed care workers. Additionally, a range of policy and practice measures have aimed to improve the efficiency of community care services, especially at their interfaces with acute health care. However, the effectiveness of all these policies is seriously threatened by a severe lack of funding for community care services, relative to both current and projected future demand." @default.
