- W37509729 abstract "Abstract To explore the possible mechanisms mediating the clinical effect of light treatment, the covariation between biochemical rhythm markers and the clinical outcome of light treatment was evaluated. Another aim of the study was to test for a biochemical predictor of the clinical outcome of light treatment. Results from 109 patients with major depression studied autumn 1988 to spring 1991 were included. On Day 1 blood sampling was done 20.00–08.00 hours measuring serum melatonin, Cortisol, prolactin and TSH. On Day 3 clinical rating with the use of the Comprehensive Psychopathological Rating Scale (CPRS) and the Hamilton Depression Rating Scale (HAM-D) was performed. The patients were exposed to bright light (luminance: 350 candela/m2) for 1 hour (22.00–23.00 hours) and blood sampling was done. The light treatment was given for 10 consecutive days in a room with indirect light of 350 candela/m2 luminance. The patients were treated with light 06.00–08.00 hours (morning light) or 18.00–20.00 hours (evening light). Of the 109 depressive patients 94 completed the treatment procedures and of the patients with winter depression treated with morning light 75% were improved. When the serum melatonin level during the light test was lower at 23.00 hours than at 22.00 hours, clinical improvement in the patients with winter depression treated with morning light was 88%. The area under the profile time curve (AUC) of melatonin decreased significantly more after light treatment in the improved. The time center of gravity (TC) of melatonin was phase advanced by light treatment in the whole group of depressive patients; 06.00–08.00 hours light induced a phase advance and 18.00–20.00 hours light a phase delay of TC. The AUC of Cortisol was increased by morning light and decreased by evening light in the whole group of depressive patients. In the improved patients a phase advance of Cortisol TC was seen in opposition to a phase delay in the nonimproved patients. In the patients with winter depression a phase advance of TC was also seen in improved patients. Morning and evening light had differential effects on the AUC and TC of TSH in the whole group of depressive patients with a decrease of AUC and a phase advance of TC with morning light. The clinical effects of light therapy seem to be related to alteration of biological rhythm markers." @default.
