- W386088028 abstract "Objective: To evaluate the relationship between the presence and severity of psychosomatic complaints and headaches in patients with psychogenic non-epileptic seizures (PNES) vs. patients with epilepsy (PWE) admitted to the epilepsy monitoring unit (EMU) for seizure evaluation.Background: Headaches and psychosomatic complaints are frequently reported by patients with seizure disorders but the relationship between these two complaints is not clear.Design/Methods: All patients admitted to epilepsy monitoring unit (EMU) on or after 5/1/2014 were eligible. 98 consecutive patients were approached and filled out all study questionnaires prior to establishing definitive seizure diagnosis. Headache characteristics [using International Headache Society (IHS) ICHD-2 criteria], demographics, PHQ-15, MIDAS, and HIT-6 questionnaires were distributed. 25 agreed to participate but did not provide data. Data from 50 adults (ages 18-74) discharged from the EMU with a single diagnosis of either epilepsy (PWE) or non-epileptic spells (PNES) were analyzed for differences in the following variables: age, PHQ15, MIDAS, and HIT-6 using t-test; regression analysis was performed to evaluate the relationship between the diagnoses and the psychosomatic measures.Results: 26 PWE and 24 PNES patients were studied. In univariate analyses, patients with PNES were older (44.7 vs. 31.5; p=<0.001), had higher PHQ15 score (8.29 vs. 3.96; p=0.047), higher MIDAS score (52.3 vs. 12.0; p=0.002) and higher HIT-6 score (63.6 vs. 58.7; p=0.061). Multivariate logistic regression model with all these factors included showed that only higher MIDAS scores were significantly associated with the diagnosis of PNES vs. epilepsy (B=0.064; SE=0.025; p=0.011). This relationship is independent of other factors included in the model.Conclusions: In patients with seizure disorders and headaches, patients with PNES report higher MIDAS scores indicating higher severity of somatic complaints and higher impact of headaches on their lives.
