- W387623930 abstract "背景．急性呼吸不全を呈するLambert-Eaton筋無力症候群（LEMS）は稀である．今回我々は肺小細胞癌に合併し急性呼吸不全を呈したLEMSに対し，化学放射線療法と3,4-diaminopyridine投与により改善した1例を経験したので報告する．症例．59歳，男性．20XX－1年9月頃から両下肢脱力感，筋力低下を自覚していた．20XX年1月に当院神経内科を紹介受診し，LEMSと診断された．CTにて縦隔リンパ節腫大を認め，縦隔鏡下生検で小細胞癌と診断された．肺内，他臓器に明らかな原発巣は認めず，LEMSを合併したcT0N2M0肺小細胞癌と診断した．呼吸筋低下から急性呼吸不全をきたし，人工呼吸管理下にcarboplatin＋etoposideによる化学療法を施行し，腫瘍の縮小を認めた．さらに3,4-diaminopyridineの投与で速やかな筋無力症状の改善が得られ，その後の化学放射線療法が完遂できた．結論．LEMS合併の肺小細胞癌において，3,4-diaminopyridine投与は支持療法として有用であった．" @default.
