- W39323198 abstract "OBJECTIVE: To report a rare case of parasylvian or operculo-insular pain at the onset of stroke. BACKGROUND: Parasylvian or operculo-insular pain has been described as a distinct central pain syndrome in the literature. These patients usually have central pain with dissociated thermoalgesic sensory loss on the contralateral side of the stroke in parasylvian or operculoinsular region. The central pain or dysesthesia with this syndrome is usually seen during recovery from the stroke. DESIGN/METHODS: Case report in a university hospital setting. RESULTS: 51 year old female with past medical history of coronary artery disease, and hypothyroidism, comes in with sudden onset burning sensation, and numbness in left hand. She has also started dropping things from left hand. On exam, she has sensory deficit in the anatomic snuffbox and volar aspect of the hand and of the 1-2 fingers in the left hand. She also has weakness of flexors and extensors of the 2-3 fingers and complains of dysesthetic sensations in the whole left hand. Neurological examination otherwise is unremarkeable. She was then admitted to the stroke unit and MRI brain showed an acute non-hemorrhagic infarct in the right parasylvian region with hyperintensity on DWI, and hypointensity on ADC sequence. Over the course of her stay, she had improvement in strength in the left hand. CONCLUSIONS: As mentioned above, dysesthesia has been described in cases of parasylvian or operculo-insular infarcts during recovery from stroke, but we present a case where dysesthesia is the presenting symptoms itself.
