- W40524757 abstract "Gliomas constitute the vast majority of primary cerebral malignant neoplasms and are highly resistant to the currently employed treatment with a combination of surgery, radiotherapy, and chemotherapy. The median survival time of patients with malignant gliomas (primarily glioblastomas) treated with surgery alone is only 17 wk. Addition of radiotherapy extends median survival to 37 wk. Over the last 30 yr, adjuvant chemotherapy has offered only modest improvement, with prolongation of survival by several weeks or a few months (1). The development of powerful tools for the manipulation of genes and vectors has enormously increased the possibilities of approaches to cancer therapy other than pharmacological agents. Treatments based on genetic mechanisms will exploit the extensive body of information that has been accumulated concerning the molecular basis of cancer. The new strategy is aimed to take advantage of the underlying genetic abnormalities of tumor cells, those that contribute to the unregulated growth of cancer cells, prevent apoptosis, activate angiogenic factors, induce proliferation, and promote invasiveness. One of the most promising new therapeutic approaches involves the transfer of genes that may irreversibly modify the neoplastic phenotype of the cancer cells. In addition to the quick progression of the molecular biology of cancer, scientists around the world are committed to translating their research from laboratories to bedside. There are now over 100 protocols in the field of cancer gene therapy in progress worldwide (2) (Table 1). The result of one such study was published very recently. This work reports the remission of lung tumors after treatment with intratumoral injection of a vector able to express high levels of wild-type p53 protein (3). Over the past 4 yr, our group has extensively studied the effect on glioma growth produced by transferring p53, p16, and retinoblastoma (Rb) tumor suppressor genes to gliomas using recombinant adenovirus. In this chapter, we will review the gene therapy strategies based on tumor suppressor genes for glioma, and will present the experience of our laboratory in this field. Finally, we will speculate on the possible future directions and improvement of this modality of treatment for brain tumors." @default.
