If the insurance industry/managed care lobby couldn't perpetuate the dichotomy between physical health and mental health/addiction, then we would truly be close to achieving parity in coverage/reimbursement. I'll give you an example. In New York State, as in many others, we have something called the UPPL law (Uniform Accident and Sickness Policy Provision Law) that allows insurance companies to deny coverage for patients presenting to an emergency department (ED) with any alcohol or drug-related injuries sustained in a motor vehicle accident (MVA). They are considered, under the UPPL law, self-inflicted. The legislation, proposed to abolish the UPPL, was defeated in both the Assembly and Senate in New York State. In sum and substance, the auto insurance industry attempted to link the proposed legislature in a way so as to adversely impact physician's reimbursement for patients injured in MVAs while intoxicated. This would have de-certified certain physicians from treating no-fault patients in general. Based on my ED experience and my years as an addiction doctor, I can state with a fair degree of certainty that if the ED staff is not asking about alcohol in MVAs then they are not asking about alcohol and other drugs in most, if not all, trauma cases. I could go on to elaborate on why it is so important to screen for alcohol and other drugs at the ED especially as the Baby Boomers grow into maturity (eg, alcohol on top of prescription meds and hip fracture?) but I'll just leave that for another time.
