• W412922372 abstract "ラタノプロストは17位にフェニル基を導入することにより,プロスタグランジンFP受容体への選択性を向上させ,プロスタグランジンF2α(PGF2α)の持つ眼圧下降作用と眼局所における副作用を分離したPGF2α誘導体である.ラタノプロストはPGF2αと同程度に強力なFP受容体作動薬であるが,FP受容体に対する選択性はPGF2αより高かった.ラタノプロスト2.5μgを高眼圧サルに点眼すると持続的な眼圧下降が認められ,その程度はイソプロピルウノプロストン60μgと比較して大きかった.ラタノプロストの眼圧下降作用はウサギあるいはネコでは認められず,これはラタノプロストのFP受容体への高い選択性と眼局所に存在するプロスタグランジン受容体サブタイプの違いに起因するものと思われた.ラタノプロストはサルにおいて房水産生量および線維柱帯流出量に影響を与えること無く,ぶどう膜強膜流出量のみを増加させた.このぶどう膜強膜流出量の増大は主に細胞外マトリックスの減少によるものと思われた.また,眼圧下降作用以外に血流改善作用および神経保護作用がラタノプロストの抗緑内障効果に寄与している可能性が示唆されている.ラタノプロストには眼刺激作用が認められず,結膜充血作用は極めて軽度であった.ラタノプロストは原発開放隅角緑内障あるいは高眼圧症患者においても1日1回点眼でマレイン酸チモロールよりも優れた持続的な眼圧下降作用を示した.また,ヒトにおける房水動態の検討でも,ぶどう膜強膜流出量のみが増大した.以上の様に,前臨床薬理試験で示されたラタノプロストのぶどう膜強膜流出量の増大作用に基づく眼圧下降作用が,臨床試験においても明らかに立証された." @default.
