• W4200059593 abstract "The rates of fertility enhancing treatments and use of assisted reproductive technology is increasing in the United States. There is conflicting evidence regarding an independent effect of infertility treatments and the risk of small for gestational age (SGA) birth weight and still birth. Our aim is to evaluate the risk of SGA infants in reproductively assisted pregnancies in a large contemporary nationally representative cohort. This was a population based retrospective cohort study of all deliveries in the United States in 2018 using national live birth and death certificate data. Multiple gestations, fetal anomalies and suspected chromosomal disorders were excluded. Patients were grouped by spontaneous conception, infertility treatments (IT: artificial insemination, intrauterine insemination or fertility drugs), and assisted reproductive technology (ART: IVF, GIFT, or ZIFT). Baseline demographics and pregnancy outcomes were compared in a univariate analysis. Poisson regression with robust error variance was used to generate Incidence Rate Ratios (IRR) and 95% confidence intervals (CI) for risk of SGA and Still birth. There were 3,387,019 births in the analysis, of these, 3,337,759 were spontaneously conceived, 18,072 were assisted with IT and 31,188 were conceived with ART. Patients undergoing IT or ART were more likely to be older, NH-White, privately insured and non-smokers. Unadjusted SGA rates were 9.3%, 9.0% and 8.1% and still birth rates were 0.4%, 0.5% & 0.6% in spontaneously conceived, IT, and ART pregnancies, respectively (Table 1). After adjustment, women undergoing ART had increased risks of both SGA (IRR 1.05, CI 1.01-1.10) and still birth (IRR 1.69, CI 1.37-2.07), compared to those who spontaneously conceived. Notably, this risk further increased in those with IT alone (SGA: IRR 1.15, 1.10-1.21) and (still birth: IRR 2.18, 1.70-2.79).
