There is limited knowledge related to maternal mortality among individuals with perinatal substance use and less known beyond the first year postpartum. Therefore, the objective of this retrospective cohort study was to estimate both the one-year MMR and two-year mortality ratio for people who use drugs during pregnancy. The cohort was drawn from the Maternal-Infant-Data-Hub, a repository of maternal-infant dyads with linked healthcare, billing, and vital records from one delivery hospital in Cincinnati, OH. Individuals who delivered a newborn in 2013-17 were eligible for this study. People with any ICD codes for substance use or a positive urine drug test were considered to use substances during pregnancy. Death certificate data was used to identify individuals who died and cause of death. The MMR was calculated in SAS for the entire cohort, among people with prenatal substance use, and among people without substance use. Two-year mortality was calculated in the same manner, with the exception of defining deaths as occurring within 2 years of delivery. A total of 11,901 live births, and 26 deaths within 2 years of delivery were identified. Drug-related deaths were the leading cause of death (51%) within the entire cohort. Overall, the MMR was 143 and the 2-year mortality ratio was 219 deaths per 100,000 live births. Among persons without prenatal substance use (n=9,458), the MMR was 11 and 2-year mortality ratio was 85 deaths per 100,000 live births. Among persons with prenatal substance use (n=2413), the MMR was 539 and 2-year mortality ratio was 746 per 100,000 live births. Drug-related deaths are a major contributor to both maternal mortality and beyond the first year postpartum in Cincinnati. Additionally, people who used drugs during pregnancy were substantially more likely to die within two years after delivering a newborn when compared to those without substance use. Studies examining mortality within this population are urgently needed to improve outcomes." @default.
