- W4200195887 abstract "To estimate patterns of alcohol and marijuana use from pregnancy to the postpartum period and to identify factors associated with postpartum substance use initiation in a large and diverse cohort of people living with HIV (PLHIV). We analyzed data on marijuana and alcohol use from the Surveillance Monitoring for ART Toxicities cohort of the Pediatric HIV/AIDS Cohort Study, which has been enrolling pregnant PLHIV at 22 United States sites since 2007. Included in the analysis were participants enrolled from 2007-2019 who had self-reported substance use data available in pregnancy and 1-year postpartum. Postpartum marijuana and alcohol use patterns were categorized as: 1) never (no use in pregnancy or postpartum), 2) initiation or resumption (no use in pregnancy, use postpartum), 3) continuation (use in pregnancy and postpartum), or 4) discontinuation (use in pregnancy, no use postpartum). Multivariable log-binomial or Poisson models were fit using generalized estimating equations to determine clinical and demographic factors associated with initiation/resumption among participants with no use in pregnancy. A total of 1774 pregnancies of 1429 PLHIV were eligible for inclusion. Among them, alcohol (38%) and marijuana (9%) were commonly initiated postpartum (sign test p< 0.001; Figure). Compared to never use, the adjusted relative risk of postpartum alcohol initiation was greater for those with younger age, higher household income, greater education, Midwest location, tobacco use during pregnancy, and cesarean delivery (Table). Compared to never use, factors associated with marijuana initiation included younger age, lower household income, more recent delivery year, psychiatric diagnosis during pregnancy, and tobacco use during pregnancy (Table). Postpartum initiation or resumption of alcohol and marijuana use are common among PLHIV, and risk factors differ for alcohol versus marijuana. Understanding the clinical and obstetric factors associated with postpartum initiation of substance use may enhance tailoring of interventions to optimize health among postpartum PLHIV.View Large Image Figure ViewerDownload Hi-res image Download (PPT)" @default.
