- W4200209044 abstract "To test the hypothesis that outcomes of sage III TTTS do not change by visibility of the bladder. This was a retrospective cohort study of prospectively collected data of consecutive TTTS cases that underwent fetoscopic laser surgery (FLS) between 2012 and 2020. Triplet pregnancies and missing records were excluded. This study focused on the Stage III TTTS which was divided into Stage III atypical (abnormal Doppler flow with visible donor bladder) and Stage III classical (abnormal Doppler flow with non-visible donor bladder) groups. Both groups were managed equally. Preoperative, operative, postoperative, and neonatal data were collected. Univariate and logistic regression analysis were performed. P value of < 0.05 was considered statistically significant. A total of 93 stage III TTTS pregnancies were identified. Of those, 59 (63.4%) had classical stage III and 35 (37.6%) had atypical one. There were no differences in maternal demographics. Preoperative data showed higher maximal vertical pocket (MVP) in the donor’s and lower in the recipients’ sacs in the atypical group compared to the classical (median: 1.65 vs0.85, and 8.6 vs10, respectively, p < 0.001). There were no differences in Doppler studies between groups. Operative data showed that arteriovenous (AV) anastomoses recipient-to-donor (R-D) were significantly higher in the atypical group compared to the classical (median: 4 vs 2, respectively, p=0.003). The frequency of intraoperative amnioinfusion was higher in atypical group (60% vs 32.8%, p=0.009). There were no differences in rates of sFGR, postoperative data, early fetal demise, and neonatal survival. AV R-D anastomoses were significantly higher in atypical stage III TTTS. This finding could have acted as a rescue to donor’s renal function, resulting in visible bladder in the donor. Outcomes were not different between groups, which emphasize on the importance of Doppler studies in evaluating TTTS. Presence of bladder and less impressive fluid discordance should not triage the Doppler assessment." @default.
