- W4200316098 abstract "Primary entry refers to the entry that caused acute aortic dissection. Lifesaving is said to be the main purpose of surgery for acute aortic dissection. Removal of the entry that causes malperfusion is the main principle of surgery for that lifesaving, and it is called a tear-oriented strategy. This issue features a fine report by Kawahito et al. [1] analysing data regarding the influence of residual primary entry following the tear-oriented strategy for acute type A aortic dissection. Their institutions are 2 leading Japanese hospitals in the medical care of acute aortic dissection. They retrospectively reviewed over 1100 patients in about 30 years and found that residual entry lowered distal aortic event-free survival rate. The primary entry exists in the ascending aorta to the arch aorta in most cases, and primary entry resection can usually be achieved by ascending aorta replacement or hemiarch replacement with a large incision to the lesser curvature of the arch. In fact, in their report, although the actual number is not known, it was shown that the entry was located from the aortic root to the ascending aorta in most cases after propensity score matching (PSM). They indicated that resected entry or residual entry was defined on the basis of intraoperative visual findings. In the cases after PSM, there were 17 entries at the aortic root or the ascending aorta, 14 entries at the aortic arch, and 221 entries at the descending aorta in the residual group. If these were visually confirmed, there is a lack of explanation as to why these were not resected with the tear-oriented strategy." @default.
