- W4200478800 abstract "Approximately half of cases of neonatal hypoxic ischemic encephalopathy (HIE) are graded as mild. These infants are expected to have normal neurological prognosis and, have neither been subject to hypothermia treatment nor the extensive follow-up as cases with moderate to severe HIE. Recent studies suggest that the outcome after mild HIE might not be as favorable as previously presumed, and more knowledge is needed. The objective of this study was to investigate important adverse outcomes at up to seven years of age in infants with mild neonatal HIE. Register-based nation-wide cohort study for the years 2009 to 2013 in Sweden. Data from the validated registers included 480 491 live births from 36 gestational weeks and beyond. Follow-up was at least 2 years, until 2015. The main outcome was a composite of a diagnosis of cerebral palsy, epilepsy, mental retardation, or death. Secondary outcomes were each diagnose separate. Mild HIE was compared with those without. Infants with a diagnosis of moderate to severe HIE were excluded from the cohort. Odds ratios (OR) with 95% confidence intervals (CI) were calculated. The incidence of mild HIE was 0.4/1000. Of the 207 infants with mild HIE, 11 (5.3%) had any diagnosis compared to 6991 (1.4%) without HIE, OR 3.80 (95% CI 2.07-6.98). Cerebral palsy was diagnosed in 2.9% vs. 0.1%, OR 24.56 (95% CI 10.86-55.55) and epilepsy in 2.9 % vs. 0.4% OR, 7.95 (95% CI 3.53-17.94). There was no association between mild HIE and mental retardation OR 1.47 (95% CI 0.47-4.59), or death, (0 vs. 0.1%). The findings of this large population-based study, indicate that mild HIE is associated with an increased risk for the diagnosis of cerebral palsy and/or epilepsy during early childhood. Our results are in line with recent studies and, whether the associations represent a predominantly fetal predisposition, or partly might be related to labor, remains to be elucidated." @default.
