- W4200496925 abstract "Determine if there is an association between the myometrial thickness (MT) of the lower uterine segment of advanced maternal age (AMA) women and mode of delivery. This was a retrospective cohort study of AMA women who underwent a mid-trimester anatomy ultrasound with transvaginal cervical length measurement from 2014-2020 at Mount Sinai Hospital. Inclusion criteria included nulliparous women, 35 years or older at delivery, undergoing an ultrasound between 18 and 23 weeks gestation. The image had to meet the C.L.E.A.R criteria for the measurement of cervical length. The MT was defined as the width observed in the lower uterine segment as obtained by transvaginal ultrasound and measured in millimeters (image 1). The primary outcome was cesarean delivery (CD) after a trial of labor. Receiver operator characteristic curves were also constructed in order to define a “thin” myometrium, and compare the area under the curve between the MT and a previously reported nomogram to test if the MT more accurately predicted the risk of CD. 624 unique scans were identified in AMA women, 221 were deemed to have optimal images and met inclusion criteria. There was no statistically significant difference in the median MT between those women who delivered via CD and those that delivered vaginally (7.3 vs. 6.9 mm; OR 1.06 [95% confidence interval 0.98-1.14], p-value=0.16). In the AMA women who attempted a trial of labor following induction of labor, the MT was thicker in those that delivered via CD as opposed to those who delivered vaginally (7.7 vs. 6.4 mm; (OR 1.14 [95%CI 1.02-1.29, p-value 0.03 (table 1)). When compared to the GENESIS nomogram, the MT performed poorly in its ability to predict cesarean delivery (AUC of 0.57 vs 0.67). In our population the MT in the mid-trimester was thicker in AMA women who required a cesarean delivery following an induction of labor. This easy to obtain measurement can serve as an adjunct to the clinical picture when counseling this patient population on their risk of CD.View Large Image Figure ViewerDownload Hi-res image Download (PPT)" @default.
