• W4205127978 abstract "Sir: We would like to thank Dr. Culliford for his positive feedback on our recent publication.1 We are pleased that he substantiated our experience with good clinical results after autologous fat transfer. We thank Dr. Culliford for sharing his modification of our described technique of autologous fat transfer in our article. He argues to also place two 0.45-inch Kirschner wires from the first metacarpal bone to the second metacarpal bone after the fat transfer to maintain the enlarged joint space. Removal of wires was performed in the outpatient clinic after 1 week. His idea of maintaining axial traction is a very good and constructive suggestion that would be interesting to evaluate in further clinical trials. Furthermore, in contrast to our postoperative protocol of 1-week immobilization in a cast, he advises his patient a period of another 4 weeks after pin removal. His idea of an internal fixation of the bones that prevents motion immediately after fat transfer makes absolute sense and is certainly a viable alternative technique, especially with the aim of providing more space for the transplanted fat and thus improve survival of the cells. This theory is interesting and promising; however, what is reasonable for the success of this therapy is not fully understood. As mentioned in our discussion, there are several potential hypotheses for the therapeutic effect. One aspect could be that the transplanted autologous fat tissue acts as a type of “buffer” or “cushion,” but another possible hypothesis could be that adipose-derived stem cells that are included in the lipoaspirate2 mediate regenerative processes. Thus, longer follow-up and larger clinical trials with different surgical techniques are necessary, and only data with a high level of evidence could be reliably used to compare different surgical techniques.
