- W4206113340 abstract "Nitrofurantoin has been in existence for more than 60 years. Despite that, its increased use only came recently when it was introduced as first line antimicrobial agent for treating lower urinary tract infections in national guidelines such as the Infectious Diseases Society of America (IDSA), Scottish Intercollegiate Guidelines Network (SIGN), and National Institute for Health and Care Excellence (NICE). This came as a result of mounting pressure on quinolones, β-lactams and the anti-folates commonly used in treating urinary tract infections. It is also used as prophylaxis in individuals with frequent episodes of urinary tract infections. The main disadvantage of nitrofurantoin is its inability to achieve high serum levels and therefore is restricted to treating uncomplicated lower urinary tract infections. It is not recommended for treating bacteremia even when caused by sensitive organisms. One of the advantages is that nitrofurantoin is known to have low resistance rates due to its multiple action sites on the bacterial cell. Repeated or prolonged use of nitrofurantoin could lead to complications such as pulmonary fibrosis, drug induced hepatitis, peripheral neuropathy as well as hemolytic anemia in a minority of patients though it is fairly well tolerated in many patients. Absolute contra-indications are; Acute porphyria, G6PD deficiency, infants less than 3 months and eGFR < 45 mL/min." @default.
