- W4206175074 abstract "Psoriasis is a multifaceted disease that can be present in many different ways. The characteristic lesion of psoriasis vulgaris is a scaly, red, thick plaque; however, variation in the time course of the eruption, the size of the lesions, the specific areas of involvement, and the degree of inflammation may be present. Patients with psoriasis often complain of pruritus, but this symptom also varies from one individual to the next as does the extent of their cutaneous symptoms and the severity. Psoriasis primarily involves the skin, but an arthropathy can also be an associated finding. In addition, the overall impact of psoriasis on patients' lives may be the most important and the most variable aspect of the disease. The variation of psoriasis at the clinical level is mirrored in the many alterations of biochemical, genetic, immunologic, and metabolic pathways observed in the skin of patients. These many biologic changes provide at best a complex picture of the pathogenesis of psoriasis. The fact that multiple pathways are affected also provides numerous potential targets for pharmaceutical intervention. Numerous treatments are available for psoriasis. The mode of action of most of these treatments is not completely understood. The manifold treatments available for psoriasis, combined with the variation in the disease itself, makes for a complicated matrix of possibilities for the clinician treating patients with psoriasis. In this article I will attempt to present a structure for rational treatment of psoriasis. Understanding the epidemiology, pathogenesis, exacerbating factors, and impact of the disease provides a framework for developing appropriate treatment goals. Categorizing the extent of disease into localized versus generalized disease facilitates treatment decisions.
