- W4206211426 abstract "Sir: We would like to thank Dr. Yamamoto for his valuable comments regarding our article entitled “Correlation between Quantity of Transferred Lymph Nodes and Outcome in Vascularized Submental Lymph Node Flap Transfer for Lower Limb Lymphedema.”1 As noted, the results of this study showed improvement of circumferential difference and a decreased number of episodes of cellulitis.1 These two measured parameters were easy to evaluate and were the two main important signs of the disease process, including inflammation, infection, adipogenesis, and fibrosis. Patient-reported outcome is another important outcome measure for extremity lymphedema. We started to use the health-related patient self-reported outcome matrix, such as the Lymphedema Quality of Life instrument, and published an article using vascularized lymph node flap transfer for extremity lymphedema in 2015.2,3 We agree with the issue regarding evaluation of bilateral lymphedema, which was seen in a significant number of patients. The purpose of the exclusion of bilateral lymphedema patients in this study was to compare the outcome of the surgical versus the nonsurgical approach using vascularized submental lymph node flap transfer. The more variables that are controlled, the better the significance that can be achieved. Therefore, the results from this study that better functional outcome is observed in patients with a greater number of vascularized lymph nodes transferred can be applied to bilateral limb lymphedema. Alternatively, bilateral limb lymphedema can be compared using the absolute value of the circumference and the Lymphedema Quality of Life instrument preoperatively and postoperatively. The lower extremity lymphedema index is appealing because of the correction of body mass index, and further studies can possibly show advantages with other lymphedema grade assessments.4 DISCLOSURE The authors have no financial interest to report in relation to the content of this communication.
