- W4206447521 abstract "Insomnia disorder has a high annual economic burden >$100 billion (B), of which ∼75% is due to indirect costs (i.e., lost productivity and accidents). This research measures indirectly the medical cost of transport (i.e., motor vehicle, motorcyclist, cyclist, pedestrian, other), home-based and public place accidents (i.e., falls) due to UCI in the US (2020 US dollar). The contribution of UCI to accidents was quantified using the PAF method. Data on fatal accidents, non-fatal injury incidence rates, and average lifetime medical costs were collected from the Web-based Injury Statistics Query and Reporting System (WISQARS™). The odds ratios (OR) of insomnia are 1.4 (95% CI [1.07;2.06], Stone 2014) for falls and 3.54 (95% CI [1.13;12.88], p=0.04, Laugsand 2014) for transport accidents. The prevalence of UCI in the US adult population was set at 10%. Uncertainty around parameters was tested. Annually, falls and transport accidents due to UCI could cause 1’303 and 6’382 deaths, 49’593 and 106’362 hospitalizations and 394’471 and 1’064’959 emergency department visits, respectively. In the base case scenario, total medical costs reached $3.51 and $10.65B for falls and transport accidents, respectively. One-way sensitivity analysis (±10%) showed that UCI prevalence and ORs were the main cost drivers ($2.5-$4.5B and $2.6-$4.4B, respectively) for falls, and ($9.3-$11.9B and $9.5-$11.7B, respectively) for transport accidents. Testing the lower bound (LB) and upper bound (UB) 95%CI values of the ORs showed that the cost estimates can vary largely for falls (UB=$1.4B; LB=$10.3B) and transports accidents (UB=$1.3B; LB=$27.6B). Overall, UCI is estimated to cost the US healthcare system $3.51B for falls and $10.65B for transport accidents. Leger 2019 estimated the annual direct costs of accidents due to sleepy driving between $139 and $152B. The AAA Foundation for Traffic Safety estimated that 328’000 drowsy driving crashes occur annually, including 109’000 injuries and 6’400 fatalities." @default.
